3.2. Approval Authority for Exceptions The results of the risk assessment per Section 2.4 of these standards shall determine the approval authority for an exception to these requirements according to the following criteria. 1. Deviations from Weatherford’s CT SOP shall have at least risk level = low. 2. Deviations from specific requirements for HPS, EPS, SS, and RCS operations shall have at least risk level = medium. The approval authority corresponding to each risk level is: 1. Low = CT Operations Supervisor 2. Medium = CT Services Coordinator 3. High = Operations Manager 4. WEATHERFORD CT PERSONNEL This section defines minimum requirements for selected Weatherford CT service personnel. 4.1. Qualifications Definitions 1. For the purposes of this section, “competency” shall mean: a. The ability to apply knowledge to routine assignments with little supervision b. Understanding of key technologies, procedures, and practices 2. For the purposes of this section, “proficiency” shall mean: a. The ability to apply knowledge to non-routine applications and problems b. Understanding of fundamental principles, underlying technologies, procedures, and practices 3. For the purposes of this section, “fluent in English” shall mean: a. The ability to speak, read, and write English b. Understanding of English technical words, equipment names, terms, and phrases specific to CT operations 4. For the purposes of this section, “Basic Safety Certification” means certification of satisfactory completion of training for all of the following subjects. a. Job Safety Analysis (JSA) b. Risk Assessment c. Basic first aid d. Working at Heights e. Confined Space Entry f. Working with Pressure g. Operation of dry chemical fire extinguishers 5. For the purposes of this section, “Specific Safety Certification” means certification of satisfactory completion of training for the following subjects, as appropriate to the job assignment. a. b. Helicopter Underwater Egress Training (HUET) c. Tropical HUET d. Arctic Survival e. Desert Survival 4.2. CT Services Coordinator A CT Services Coordinator shall meet the requirements in this section. 4.2.1. Responsibilities The responsibilities of a CT Services Coordinator shall include: 1. Meeting with the customer personnel as required to discuss current operations and planning for future operations 2. Preparation of Weatherford’s Job Program 3. Assisting the customer personnel as required to incorporate Weatherford’s Job Program into the customer Job Program 4. Maintaining a current copy of the contract with the customer and ensuring that Weatherford complies with all requirements of the contract 5. Ensuring all Weatherford’s equipment, containers, and slings have current certification required by the customer and regulatory authorities 6. Ensuring all Weatherford’s equipment and services applied to the job meet both regulatory and the customer SH&E requirements 7. Ensuring all Weatherford’s equipment and services applied to the job comply with these standards 8. Testing and inspecting equipment and/or materials 9. Monitoring daily activities performed by all Weatherford personnel assigned to the job 10. Verifying all required data submittals to the customer are being provided 11. Assisting Weatherford’s Quality Representative in handling Quality Assurance/Quality Control issues, including responding to and resolving any non-conformance report (NCR) generated by the customer 12. Reporting continuous improvement efforts and key performance indicators (KPI) 13. Preparation of Post-Job Reports 14. Providing invoices and resolving invoice issues 15. Keeping abreast of the customer schedules and operations associated with planned or ongoing CT jobs 16. Maintaining an effective communications interface with the customer and other contractors to ensure timely progress and effective performance 17. Preparation of RFE (as required) 4.2.2. Qualifications A CT Services Coordinator shall meet the following qualifications. 1. 2. Proficiency with Weatherford’s quality management system (QMS) 3. Proficiency with CT job planning 4. Competency with computer-aided job design and evaluation 5. Three (3) years of experience with Weatherford 6. Fluent in English 4.3. Technical Support Personnel Technical Support Personnel shall meet the requirements in this section. 4.3.1. Responsibilities The responsibilities of Technical Support Personnel shall include the following. 1. CT string design and job planning 2. Preparation of Weatherford’s Job Program 3. Participation in risk assessments 4. Qualification of Weatherford’s riser and lubricator sections (as required) 5. Equipment failure investigation and analyses 6. Preparation of Post-Job Reports 4.3.2. Qualifications Technical Personnel shall meet the following qualifications. 1. Basic Safety Certification 2. Proficiency with Weatherford’s quality plan (QP) and CT SOP 3. Proficiency with Weatherford’s equipment, tools, and services applied to the job 4. Proficiency with CT string design and job planning 5. Proficiency with computer-aided job design and evaluation 4.4. CT Operations Supervisor A CT Operations Supervisor shall meet the requirements in this section. 4.4.1. Responsibilities The responsibilities of a CT Operations Supervisor shall include the following. 1. Supervising all Weatherford activities at the job site 2. Maintaining an effective communications interface with the customer and other contractors at the job site 3. Ensuring Weatherford’s equipment, services, and activities at the job site comply with these standards, regulatory requirements, and the customer Job Program 4. Monitoring daily activities performed by Weatherford personnel at the job site 5. Participation in risk assessments 4.4.2. Qualifications A CT Operations Supervisor shall meet the following qualifications. 1. Basic Safety Certification 2. Specific Safety Certification 3. Proficiency with Weatherford’s QP and CT SOP 4. Proficiency with operating Weatherford’s CT equipment at the job location 5. Competency with routine maintenance and minor repair of Weatherford’s CT equipment 6. Competency with all aspects of the computer simulations 7. Competency with operating the DAS 8. Certified in well control for workover operations by a source accredited by the International Association of Drilling Contractors (IADC) or acceptable to Weatherford 9. Certified in advanced first aid and CPR by a source acceptable to Weatherford 10. Certified to operate a self-contained breathing apparatus (SCBA) by a source acceptable to Weatherford 11. Certified for sour service operations by a source acceptable to Weatherford 12. Two (2) years of experience with Weatherford 13. Fluent in English 4.5. Equipment Operator An Equipment Operator shall meet the requirements in this section. 4.5.1. Responsibilities The responsibilities of an Equipment Operator shall include the following. 1. Operate Weatherford’s CT equipment at the job site 2. Routine maintenance and minor repair of Weatherford’s CT equipment 4.5.2. Qualifications An Equipment Operator shall meet the following qualifications. 1. Basic Safety Certification 2. Specific Safety Certification 3. Competency with Weatherford’s QP 4. Proficiency with Weatherford’s CT SOP 5. Proficiency with operating Weatherford’s CT equipment at the job location 6. Competency with routine maintenance and minor repair of Weatherford’s CT equipment 7. Maintenance Personnel Maintenance Personnel shall meet the requirements in this section. 4.6.1. Responsibilities The responsibilities of the Maintenance Personnel shall include the following. 1. Maintenance of Weatherford’s CT equipment 2. Repair of Weatherford’s CT equipment 3. Assist Equipment Operators and CT Operations Supervisors to diagnose and repair problems with CT equipment 4.6.2. Qualifications Maintenance Personnel shall meet the following qualifications. 1. Basic Safety Certification 2. Proficiency with Weatherford’s QMS 3. Proficiency with Weatherford’s preventative maintenance program (PMP) 4. Proficiency with maintaining and repairing Weatherford’s CT equipment 5. Competency with operating Weatherford’s CT equipment 4.7. Quality Representative A Quality Representative shall meet the requirements in this section. 4.7.1. Responsibilities The responsibilities of a Quality Representative shall include the following. 1. Development and satisfactory implementation of Weatherford’s local QP and CT SOP as required by the customer 2. Handling any NCR generated by the customer against Weatherford’s service operations (including equipment failures). 3. Provision of Weatherford’s corrective action response(s) and preventative action response(s) as directed by the customer 4. Actions to ensure all required corrective and preventive actions are fully implemented 4.7.2. Qualifications A Quality Representative shall meet the following qualifications. Personnel Training In accordance with Weatherford’s QMS, Weatherford shall identify and document all training needs and provide training for all Weatherford’s CT personnel to ensure they meet pre-established qualification requirements. The training requirements shall include the following. 1. Basic Safety Certification 2. Specific Safety Certification, as required for the position 3. Safety, health, and environment (SH&E) 4. Well control procedures and operations 5. Quality systems indoctrination and familiarization 6. Unique job skills/requirements training 4.9. Personnel Documentation Weatherford shall maintain documentation demonstrating that Weatherford’s personnel are properly trained and qualified for the following activities. 1. Performing required duties 2. Safely handling all fluids planned for the job 3. Operating any safety equipment required for the job 5. CT SERVICE CATEGORIES This section defines service categories for Weatherford’s CT operations. In these standards, normal circulation is flow down the CT string and return flow up the annulus around the CT. 5.1. Pressure Definitions This section defines pressures used to determine requirements for PCE and surface piping. 5.1.1. Maximum Anticipated Surface Pressure (MASP) Maximum anticipated surface pressure (MASP) at the wellhead is the maximum pressure available to the wellbore from exposed formation(s) minus the lesser of the following. 1. The hydrostatic head of producible reservoir fluid(s), or 2. The hydrostatic head of fluid(s) pumped through the CT If the producible reservoir fluid is unknown, then use a methane gas gradient (0.1 psi/ft or 2.26 kPa/m).
